NOW OFFERING VITAL STATISTICS SERVICES AT OUR LEESBURG LOCATION 

Eustis, FL– The Florida Department of Health in Lake County (DOH-Lake) is now offering vital statistics services every third Tuesday of the month from 9 am to 12 pm at the Florida Department of Health in Lake County Leesburg located at 2113 Griffin Road, Leesburg, FL 34748.  You will need to bring with you a valid state issued ID and cash or credit card. 
 
The fees are:  Birth Certificate           $15.00 ea.       Additional        $10.00
                     Death Certificate         $12.00 ea.
                     Vinyl Protector            $  5.00 ea.

We will continue to provide same day services from 8 am to 4:30 pm Monday through Friday at our following locations:
